Key & Access Device Request Form
*Key request form must be filled out in full with an Authorizing Authority Approval Signature, and sent to the Facilities Management office for processing. Forms not filled out in full will be returned to sender causing a delay in processing.
Deposits & Fees:
- Deposit - 1st Access Device $35 (Students Only - refunded upon return of devices)
- Deposit - Additional Access Devices $10 ea (Students Only - refunded upon return of devices)
- Fee - Lost Device Replacement $30 ea (non refundable)
- Fee - Rekeying (non refundable fee varies in each area, contact Facilities for costs
Term Extension Requests for Access Devices:
- Extension requests within 30 days of a term end date will only require an email from the Authorizing Authority of the room(s) originally requested
- Extension requests outside of the 30 days of a term end date will require a new Access Request Form submitted with Authorizing Authority Signatures
