Promoting patient safety through interprofessional collaboration

On March 28, a special Interprofessional Medication Reconciliation Event (IP MedRec) was held at various post-secondary sites throughout the province, including Prince George, Victoria, Kelowna and Vancouver. Hosted by UBC’s Faculty of Medicine, Faculty of Pharmaceutical Sciences and School of Nursing, this year marked the sixth annual iteration of the event.

This landmark educational session draws over 750 participants each year from UBC pharmacy, nursing and medicine (including the Northern Medical Program), as well as UNBC Nursing students and nursing students from UBC-Okanagan and the University of Victoria. The event is also open to practicing pharmacists, with eight pharmacists from community sites in Victoria, Kelowna and Prince George participating this year.

Medication reconciliation is a process by which members of a patient’s multidisciplinary healthcare team (such as doctors, nurses, pharmacists and others) collaborate to identify a complete list of medications the patient is taking as they proceed through transition points in care.
